When do IM programs require ECFMG certification?
Of 654 Internal Medicine programs we read for the 2027 cycle, a minority publish their own sentence about when ECFMG certification has to be in hand. Where a program does say, the answer is almost always earlier than you would guess: 171 state it is required at application, and only 2 push the deadline out to the start of residency. A small number phrase it a third way, "by matriculation date," which we have not tallied here because it does not cleanly sort into either bucket above; we would rather leave it unquoted than force it into a count it does not belong in.
Two phrasings, one big gap between them
"ECFMG certification required" and "at the time of application" describe a program that wants your certificate finished before you have even submitted through ERAS. "Must be secured prior to the commencement of the Residency program" describes something else entirely: a program that only cares that certification is done by the day you start, months after the Match itself. These are not close variants of the same rule. A program in the first group can screen an uncertified applicant out of consideration before an interview is ever offered; a program in the second group is stating a condition of employment, not a condition of eligibility to apply.
Why most programs say nothing at all
ECFMG certification is not mainly a program-level rule. ERAS lets an IMG apply without the certificate in hand; what a program sees is an ECFMG status report. The NRMP is where the hard deadline lives: to be matched, an IMG has to have passed the examinations ECFMG certification requires by the rank-order-list certification deadline, and the certificate itself is a condition of starting training. A program that stays silent on timing is not leaving a gap in its requirements; it is leaving the timing to those two bodies. Read ecfmg.org and nrmp.org for their current rules, which change from cycle to cycle. We do not restate them here, only what programs additionally choose to publish.
That is what makes the 171 worth reading. A program that writes "required at the time of application" is not repeating the pipeline rule, it is tightening it. The NRMP's clock runs to the rank-list deadline in the new year; this program's clock runs to the day you submit in September. Only the 2 that say "by the start of residency" are restating what would have been true of them anyway.
Why the 151-vs-2 gap should change your timeline
If you are still finishing Step exams or waiting on documentation, a program's rank-list deadline is not the only clock running. A program in the 151 that requires certification at application is telling you, in effect, that your certification clock needs to finish before your ERAS submission clock does, which for most applicants is months earlier than the NRMP's own rank-order deadline. Treating every program as though it followed the 2-program pattern, where certification can wait until residency begins, is the mistake this guide exists to prevent: the majority pattern among programs that say anything at all is the earlier one, not the later one.
What ECFMG certification actually is
ECFMG certification is the credential that confirms an international medical graduate's medical school credentials and the examination record that US graduate medical education requires before a program may train that graduate. It is issued by ECFMG, not by any program, and it sits underneath every IM program's own eligibility rules rather than replacing them. A program's page can be silent about visas, about years since graduation, about every other field this site tracks, and ECFMG certification is still, separately, a real condition standing behind the whole process. That is a large part of why so few of the 654 programs we read write their own sentence about it: for most of them, the deadline they would set is the one the NRMP already sets.
What we do not publish
We do not print a count of programs that say nothing, because "says nothing" is not itself a verified fact about a program's requirements; it is the absence of one, and absence is not a number we pin the same way we pin a quoted sentence. What we publish is exactly what a program's own page states, quoted and dated, and nothing beyond it.
